Qualifying Conditions
Not every medical condition qualifies a patient for the use of medical marijuana. Here are some of the common medical conditions that are eligible for medical marijuana use:

Completing the Online Application
If you meet the qualifying conditions, you can then fill out the online application. To complete the application, you need to following information:- Practitioner certification form.
- Any applicable fees.
- Your New York state driver’s license or non-driver ID card number.
- Information about your certified caregiver.
- Caregiver card fee.
Once You have Your Medical Marijuana Card
Once you have your medical marijuana card from the state, you can make an appointment with a dispensary. Be sure to bring your medical marijuana ID card as well as a state-issued ID card with you to this visit. During your visit, you will work with a patient care representative to complete your intake form. Once the intake is complete, you can receive up to a 30-day supply of medical marijuana. Please be aware that due to federal banking regulations, dispensaries cannot accept credit cards for medical marijuana payments. Please bring cash or a debit card to make your payment.
